Our Palliative Care Coordinator is responsible for coordinating the palliative care service plan. This includes assessing the patient’s needs and identifying a programme of care and support using a holistic approach.
This support may include:

Emotional and psychological

Education and advice on symptom management

Education and advice on nutrition, bowel care, skin and wound care and any other concerns

Communication with GPs and other health professionals involved in their care

Assessment of needs and loaning of equipment to aid in home care
Personal Care & Home Help
Following Nursing assessment, patients may be referred for Personal Care and/or Home Help assistance. This is provided by the Home Support Agencies such as Healthcare NZ, DRC or Te Puna Ora o Mataatua via the Bay of Plenty DHB.
Equipment
Specialised nursing equipment is available for loan, free of charge, following a nursing assessment. This equipment is to aid home care and includes such items as: Electric beds, LaZyboy chairs, Commodes, wheelchairs, etc.
Respite Care
We have two beds available at Golden Pond Rest Home for Respite and End of Life care. There is also the possibility of funding for a bed at other Rest Homes in the Whakatane District for those patients wishing to take Respite out of town in areas.
